The prescribing clinician's documentation is usually central, so the request should be organized enough for the office to respond quickly.
- Medication name, dose, and prescribing clinician.
- Plan denial or pharmacy rejection text.
- Tried alternatives or contraindications.
- Deadline and submission route.
