Separate the two record types
Clinical records explain the care. Billing records explain how the care was submitted for payment. An insurer may need one, the other, or both.
When follow-up gets stuck, ask whether the issue is diagnosis, procedure code, provider identifier, service location, medical necessity, or proof of payment.
Request the correction from the right office
The clinician's office may control medical records, while the billing office controls invoices, superbills, and claim forms.
Directing the request to the right team can avoid repeated denials for the same missing item.
